Check accelerator cable ...maybe it is so loose that even after pressing accelerator to the bottom, butterfly of throttle body is not opening to the full.....
If this does not solve the problem next cheapest step is to go for tuning and compression test...if you car is loosing too much compression engine will not be able to generate its rated horsepower..
If compression is OK then it may not be a big problem....just tuning and checking coils would do it..... next step would be to check injectors....

Get the clutch/ pressure plate checked, looks like it has worned out. Dose your car hesitate climbing a steap road?

might be the issue with clutch pressure set and also it can be engine itself the compression testing is the best option .

Sorry Bro...i am Lahore..unfortunatley don't know anything about Islamabad......
I have just suggested few steps...with cheaper things first...
For accelerator cable and tuning most on the mechanics would be able to do...no big science...just make sure when during tuning mechanic cleans throttle body...DONT ALLOW him to fiddle with throttle stop screw.....
For compression test, it is also quite simple but you need to go to the person who has required equipment...
As for clutch and pressure plate...test is very simple....go to speed of about 20-30kmph and put cat in 4th or 5th gear and press accelerator..if car jerks heavily and starts to stall...yout clutch and pressur plate is fine...if you feel an increase in rpm without increase in speed then it is faulty...

Check floor mat slipping under accelerator pedal, pedal not full press remove floor mat, and check clutch plate.

@iamadee has described well. If it is having jerk in gear shift then Clutch'/Pressure plates can be culprit. Before doing that bleed your clutch fluid to release any vacuum if exist.
