The 2TR is generally very trouble free and does not require special care. Just like you would for any used car, first preference should be given to vehicle with low mileage and maintenance history. I would look for a 15 or 16 model car since 15 on-wards Indus introduced climate control in these vehicles. You can also find TRD models in 15 and above but it's only cosmetic and nothing else so wouldn't matter much.
Since this model, unlike the newer shape, is a full time 4WD SUV, it's quite heavy on fuel. I wouldn't expect anything more than 6 to 8 kmpl mileage.
Apart from that, being a 4WD maintenance is a bit on the heavier side.
5.6L engine oil with every filter change
Propeller shaft lubrication with a Lithium-based NLGI 2 grease every 5000km or 6months. (practically on every oil change)
ATF change 40K km or 2 years
Transfer case oil 40k km or 2 years
Front diff and rear diff oils at 40K km or 2 years.
Nevertheless, this is just to give an idea on the differences in maintenance from a car and you can read specifics about maintenance schedule in the owner's manual.
This vehicle truly shines when you take it off-road. I've driven it on the beach sand and rocky terrain and it never disappointed. Coming from a 10th gen civic you would surely miss the agility and overall better driving dynamics on highway. But then again, both vehicles have their merits. The Fortuner can carry more people and take them to places a civic can only dream of.
