just one suggestion.. no need for the "age group".. as we have already mentioned the date of birth n others can/shud calculate it themselves... reminds me of the employment form i was filling today asking for the date of birth, n then in the next line the age in years/months:s... similarly for the job experience... (start date/end date/ and then the total time)... (maybe coz it was a government organisation:P)
