Apparently it can be obtained from Pakistan. I spoke to the original poster about this and he shared the following:
"Carnet de Passage fee is 40,000rs.
in our case we have toyota corolla 1.8, mkt value 18lac and will be depositing
16lac's against the car value to -> M. H. Khan, Karachi Automobile Association
0300-3333571. and the deposit will be paid back 2 months after our arrival
back to karachi. there office is located at II Chundigarh road. behind Alfalah
Corporate office, Writer chambers mein. Room 15-D"
