Child Birth Registration Certificate

I have recently moved to Lahore and living in Askari 11, want to get the child birth registration certificate of for my newborn who was born in Shalamar Hospital, Shalamar Link Road Lahore.

Can someone please help me in getting the exact process for child birth registration. What I know is that i will need to go to the union council office but dont have any address or details.

Child birth certificate from hospital,
Photo copy of Mother's CNIC
Photo copy of Father's CNIC
Photo copy of Electricity / Gas Bill as a proof of residence.
Certificate fee.

in order to get B form from NADRA , all you need is a birth certificate from your hospital.. where baby was born.
NADRA does not require registration form of union council anymore..... I assume your ultimate goal is to get the B form or is there any other requirement for union council registration ??

Nadra issues birth certificate from local union council ofc.

I live in Rawalpindi and my both child were born in Shifa islamabad, I have also taken Child Registration Certificate(CRC) from Nadra and Rawalpindi address is mentioned on CRC then from where I will get Birth Registration Certificate either from CDA or Union Council ?
