Procedure for Special, Disabled persons NADRA card

Well I need this information for a cousin for getting a disabled person NADRA CNIC
Presently has a normal person CNIC but to get bonafied benefits, needs to get a renewed disabled person NADRA CNIC
What all information you guys can provide me
Domicile is from Attock but currently residing in Rawalpindi I know we need to submit Original ID card n NCRDP certificate (from some provincial or district social welfare office)
we went to SOcial welfare office in DHQ Hospital Rawalpindi
the guy Mr Saleem over there said that since the domicile n permanent address is from Attock so the case needs to be processed from there
i personally persuaded him that even provincial social welfare office can issue this certificate but he was adamant that the case needs to be processed from Attock
so i need an informed guidance
before i leave for attock

NADRA needs two things
Old CNIC n Valid Disable certification on NCRDP certificate
i am asking does one has to get this certification done from District of their Domicile only???

n by the way asking on forums is for seeking help from some one who had ist hand experience doing all this
Pakistan main Govt nay institutions tou bana diye hain but they always tell u such a path which is time consuming n lengthy in short a wrong advice for a work which is their own responsibility

NCRDP is now dealing with Federal areas only.
You may go to nearest District Headquarter Hospital (Attock or so) and met with Social Welfare Officer there. Fill the form, submit there and get a date for medical board. After clearance from medical board you will be issued disability certificate. submit this certificate alongwith your CNIC and get special persons CNIC from NADRA after few days.

were u successful in preparing the cnic or not?.......im from pindi need to prepare it for my brother who is disable

The Divisional Health officer of the place of domicile will issue the certificate of disability. with that certificate visit NADRA office with B form or the previous CNIC of healthy days.

i went to DHO rawalpindi, his staff told me that i had to go to DHQ rawalpindi that is in fawara chowk rawalpindi and DHQ will prepare it.....confused.

whats' the confusion ? DHO works under District Head Quarter & guided you to the competent authority which will surely ask you to get some verification done by DHO. be prepared to shuttle few time between both places. Good Luck !.

so after lot of khajjal khwari,,,,,i was on track,,,,,here is the correct procedure im posting so that all can benefit...

1. step 1 is to purchase form for initial processing....( i purchased from DHQ hospital located at fawwara chowk pindi)
2. step 2 is to fill the form and submit it to social welfare office....( i submitted it to social welfare office located at chowngi no.8 near khurram colony/sadiqabad). at this stage concerned staff will ask the medical documents of the disabled person showing the disabality.
3. step 3 , medical board will call the disabaled person on a given date at DHQ hospital.
4. step 4, after a month or so u will get the certificate which u can take to NADRA office and start the procedure for CNIC...

Then you need medical certificate ( you can get from g8 national rehabilitation islamabad )

Your parents id card ( photo copy )

Registery of your house in islamabad ( copy of registery )

Perivious medical record ( original )

Your id card ( copy )

Then you to go NCRDP office in f7 markaz beside shell pump ( take your all documents with you )

They give form which you have to fill and attach all the document with the form

Then they give you board date ( board set 1 time in month )

After that you set NCRDP certificate
