Excellent writeup on the starter problems.
Id like to add to the list here (though you covered it a short while back) that the battery to starter cable may be old and have resistance built up along the cable and/or any of the connectors contributing to the start-up issues. Thus, like you already did, replacing the battery to starter cable is thing to do too.
