fan is direct? get it automatic.
When cold, start the car and remove the radiator cap. check if there are water bubbles or water is being thrown out (difficult to check if the thermostat is there restricting the flow) but if you see either of the issues, you have a weak or blown gasket.
If not, get your radiator serviced, put in a good coolant like Toyota genuine coolant (orangish pink). check the radiator cap and the reservoir for any leakages as well.
There could also be problem with water body. If you have been using only water, there are chances that the fins of the water body have been corroded and it needs replacement. an expensive but necessary repair.
