hey, first of all always put a 50/50 mix of high octane & regular premium. secondly, after what you have already done, best bet would be to unscrew your injectors and get them checked. if necessary get them changed. I would still say recheck your fuel tank, many at times the already rusted tank does not hold good for long. Note that if you have cng installed and run your car mostly on cng, then it could be the sludge built up in your cylinders as well. In that case, have a boroscopic inspection of all the pistons heads and cylinders and get them cleaned if necessary. Thanks
