You pour engine flush fluid in the engine by opening the engine oil filler cap. You start the engine and let it idle for at least 15 minutes, during this process, you NEVER drive your car. After 15 minutes, you take off the engine oil drain bolt, and let engine oil (which contains engine flush material, sludge / gunk, but depending, etc.) drain. After that, pour in the new, fresh engine oil, and enjoy driving' 
And an important thing: After engine flush, change of oil filter is a MUST!
Going for engine flush is a personal choice, its preferable thing but not compulsory, and I usually recommend it when going for change of engine oil grade / viscosity (may be engine oil brand too), so as to remove everything related to old engine oil grade (may be sludge / gunk too).
