It is a single right source camera right-up.
Looks to be the pattern of a softbox but it's difficult to tell. If this were a high resolution image I could zoom into the eyes and guess the light source in a better manner.

Yeah absolutely, this shot is a piece of cake.
Take a shot in AV mode, note down the settings and then go into Manual mode to tinker the settings to get the result you want.

Try Canon link in Blue Area or the Nikon shop(don't remember the exact name) in F-6 on next to Shaheen and D-Watson. In my previous experience they get me stuff on order, mainly lenses and tripods. Worth a try.
