Problem 1:
The shoulder pain you are talking about can be due to multiple causes, so lets try to deal with them as follows:
Is their neck pain or feeling like that neck pain is radiating to the shoulder or arm? If yes then this is most probably nerve compression in the cervical area. This is investigated using plain x-ray first then an MRI. Treatment is neck exercises along with pain killers and muscle relaxants and physiotherapy.
If there is only shoulder pain and as you are describing with no backward or upward movement. Then this could be either frozen shoulder or rotator cuff impingement also called rotator cuff tendinitis. So both of these are extremely difficult to distinguish the thing that does it is a lidocaine injection in the area will cause relief in rotator cuff tendinitis whereas in frozen shoulder it will do nothing.
But as I can see this is looking more like frozen shoulder as she has many risk factors for it like age>40, female gender, diabetes. So for both X-ray can be done and then an MRI. Problem 2:
The pain in the upper calf and popliteal area again can be many things, so lets start:
Is their any back pain or is the back pain radiating to calf? if yes then it is compression of nerves in the lumbar vertebrae. This is investigated using straight leg raising test can be done at home watch a video on the net and then plain x-ray first then an MRI. Treatment is back and leg exercises along with pain killers and muscle relaxants and physiotherapy.
If it is only present in the upper calf and popliteal area then this can also be a muscular strain or baker cyst or DVT. For baker cyst a protrusion can be felt in the popliteal area. For DVT it is really difficult to diagnose without any tests like d dimer, ultrasonography. For muscular strain it can be easy if there is relief with muscle relaxant and pain killers then thats it.
I would advise you one thing in the end. As she has age > 60y kindly get a DEXA scan done to find out osteoporosis. If dexa scan confirms osteoporosis then she would need an yearly injection of zometa along with daily calcium and vitamin d. Because osteoporosis is very common throughout the world and mainly affects the spine so these problems of her can be due to it.
Then what is her weight is the BMI in ideal range if not then decrease weight.
