Considering Japanese and Chinese eat raw fresh cockroaches and lizards and what not, this is not surprising. A matter of perspective.
And as someone pointed out above, this is the cockroach that they forgot to remove. Most other restaurant cooks & waiters are more intelligent that at least before serving, all 'unpleasant stuff' is removed. As with everything, eating outside has its pros & cons.
