The jerky behaviour at 40km/h during mild cruise is the TCC fully locking in, It cannot be fully removed as its a feature, you will also notice that when you release the throttle after a strong burst of initial acceleration, the car will lunge forward then start to slow down. This is also normal for CVT - as its the TCC fully releasing.
If you are experiencing this too much then check the engine mountings. the engine should be pretty solid in the car - and not flopping about. If that it ok, and you are also seeing correct apply pressures then I think your primary valve body is acting up.
I do repair these now - mostly JATCO, but the failure you have mentioned also has showed up with a failed sheave pulley. (I hope its not that for you, as that repair is very expensive)
