You can't apply for smart card online, you have to visit Islamabad Excise & Taxation office in H-9 and there you have to submit your old registration book with some other basic formalities and then you have been given a date for the collection of the smart card.

I mean the usual paperwork, like filling out some forms and all.
I suggest you to go to Excise office in the morning around 9 am as in the morning, there are few people and you'll get free soon. You will get the complete information from the Information Counter near the Entrance gate.
